Questionnaires

[1] The instructions said to tick ONE box only. But some people have ticked 2 boxes

[2] We used an existing question on quality of life which requires adding 5 questions together and getting the answer, but some people have not filled out all 5 questions.

[3] Some people have not ticked boxes but have written comments down the side of the questionnaire

	a)	Dealing with [1] - We need to agree as a team how we are going to mark questionnaires which have 2 boxes ticked instead of one. Always take the most severe OR flip a coin for which one to choose OR agree to put missing data here? As long as we are consistent and always do the same thing then it is ok
	b)	Dealing with [1] - If people tick two boxes then just pick one answer and use that.
	c)	Dealing with [2] - Take the average of the questions that the person did complete to give the quality of life score
	d)	Dealing with [2] - Record this as incomplete and missing data
	e)	Dealing with [3] - Pick the answers out of the of the comments as best as we can
	f)	Dealing with [3] - Write back to the person and ask them to complete the questionnaire properly
